Or am I just bad at aiming some how? I put the white dot right on their heads, fire, and it completely misses?

The sights are misaligned, just like on the standard hunting rifles. Shots land to the right of where you're placing the dot on the sight. I think it's a mesh issue, IIRC.
Not to be mean to the developers, as I'm sure they worked hard on this game. But why do the big time dev teams always mess stuff up that a single 16 year old in his basemant doesn't? I'm sure within a week of the DLC comming out modders had a fix for this issue, it's a couple of years later, and no offical dev fix yet? How can they just let a major function like this be bugged? It just blows my mind when devs miss this stuff. "Oh, btw, iron sights in our first person shooter don't work." "Oh, btw, the phoenix class can't transform into a phoenix, our bad."
So, can I get a scope for this gun then? Would it fix this problem if I did? The gun cost 17,000 caps... That's one hell of an "oops" by the devs.
I never saw Hunting Rifle weapons as 'misdone', but it's a 200 year old battered rifle, of course its sights would be off for various reasons. It could be that it's just misdone, but whatever.
There's only scope for the normal hunting rifle, not the Paciencia. Or just get Christine's COS Rifle. It has better damage, better crit damage and better crit chance than all weapons that use .308 rounds.
Also... it has a silencer.
Else, just turn off iron sights. Many of the iron sights are completely unreliable. Like the cowboy repeater's iron sight... It makes the user see through this tunnel vision that completely block everything else around him. It svcks utter balls at moving targets. And the 12.7 SMG which already has WONDERFUL accuracy and TOTALLY low recoil ... which gets nuttier with the iron sight that is a wide as a brahmin. Unless using VATS, that thing fails in close-quarter stealth.
