There is one railroad that runs in a Very Hard locked tunnel, it's west of the BOS Safehouse.

If I recall, it's easier to come in from the train tracks coming from the Boomers Nellis base. Hook a left coming out of the front gate and follow the tracks. When the tracks split, stay to the left.

There are sewers but not subways probably because there aren't any subways in the real vegas.

There's a sewer system that runs all around vegas. Look for manholes in front of Camp McCarran and the Crimson Caravan, among others.

There's even people living down there.

and "The Thorn" is in a sewer, where you can get the quest "bleed me dry" = http://fallout.wikia.com/wiki/Bleed_Me_Dry
