Psychopath or not, I see no reason to poison an entire orphanage just cause I have a poison vial in my hands.

But is there any reason for my psychotic character to do it?

So poisoning them is pointless, even for my psychotic character.
And a psychopath character is idiotic in any of the games.
Why would a psychopath help NCR, House, Yes Man, Caesar, Dad, Lyons, Eden, Overseer, Master or Arroyo Elder?
An insane psychopath character with no rhyme or reason for anything can't logically complete the game.(S)He'll just run around being lol-evil everywhere.
There's no point in having lol-evil cause being that kinda psychopath makes no sense with the main story. (Possibly Yes Man but that's only cause I haven't finished Yes Man's story yet and don't know how it plays out.)

I think you hit the nail on the head right there. I have tried to make a few EVIL characters in both FO3 and FONV, and somehow along the way i either get bored or the choice to be evil is so [censored] stupid that to do so would be completely moronic that it just goes against any of my character builds. One does not survive in a waste land being stupid for long the attrition rate is to high, so to just be stupid evil is a way to naturally select one's self from the gene pool. I think the issue is to provide more depth in the choices. I think choices do not necessarily have to be gray only, if you could provide me with a good, bad and a gray solution and tie the consequences to each one would i think be the holy grail of writing. For example in FO3 you could help the slavers, destroy the slavers or ignore them, but that is not really enough is it. Instead of destroying the slavers how about enslaving the slavers as your gray choice, so the player could destroy the slavers for a good kharma boost. The PC could work for the slavers for a bad option with the gray of enslaving the slavers meaning that folks good or bad would not completely trust you. Granted i am no writer but i think with a liberal dose of imagination you guys can probably see what i mean.
But in the end Gabe i think your right though that the game does not seem to be able to capture that "truely evil bastard" feel so in the end no matter how many puppies i have bagged and tossed in to a river somewhere along the way the game presents that so totaly assinine stupid evil choice that to excercise that choice would to a degree be equal to the PC putting a gun to thier head and pulling the trigger just because. Pointless stupid evil is boring, which i why for the most part i just do what feels right which is why most of my characters are middle of the road as i think most folks are, the rest are just plain insane.
