I'll just spoiler all of these to be safe
Film: Passion of the Christ. Whole movie essentially, but particularly the whipping scenes (many a tear was shed).
Game: Dragon Age: Origins- Morrigan leaving because I refused the ritual. Less 'I liked her character' and more 'CRAP SHE HAD MY GOOD MAGE GEAR'.
Book (that I can think off at the moment): Climatic event in A Game of Thrones on the steps of Baelor. (you know the one I mean fellow Ice and Fire fans)
When Ash is turned to stone in the first Pokemon movie.
When Simbas dad is killed by the buffalo in Lion King.
Every part of Silent Hill 2 that had Angela. Mainly the parts where she is looking in a mirror holding a knife and the part where her and James are on a burning staircase. In fact, that character's scenes are what drove me to get help and actually be serious about it. Not assume there was no use in getting help for my depression or think I wasn't worth the time. It was like I was seeing a glimpse of what could happen if I didn't get help.

I didn't go through the trauma she did, but her reactions to the inner pain were so much like mine. I can't say I've played a game that actually helped convince me that there was hope except for Silent Hill 2.

Call of Duty 4: Modern Warfare: The Marines getting Nuked while looking for a terrorist leader.
Call of Duty: MW 2: No Russian: This was the most stunning event I ever encountered in a video game.
Call of Duty: MW3: When Soap died: It's a Rarity that a game makes me speechless but that moment caused me to freeze for a while, I mean you play as him in the first two MW games and then seeing him die is like watching your combat buddy or best friend passing away right in front you...however the scene that followed that was a real treat.
Bioshock Infinite: This was the best game I've played since Injustice: Gods Among Us.
Duck Tails Remastered: Although I haven't played it, I've watched LP's on YouTube and it's wonderful.
Tom Clancy's EndWar: Probably the only RTS type game I've played, using the voice command really brings out this game.
In the book Flowers for Algernon. Pretty much any point where Charlie described his treatment by his coworkers, or his peers. He had a mental deficiency in the book and people treated him so terribly. What made it all worse was how he didn't even know they were being mean to him, and he just laughed along with them.

I get feels way too frequently to remember anything in specific in movies/games/books. The ending of Shawshank Redemption is great, though.
I may strongly dislike CoD multiplayer and gameplay but I did owned the first Modern Warfare and I do agree... The marines getting nuked was a bit sad... That and the ending. Hm. Though I'll admit this, I do like the storylines of the Modern Warfares (From what I've seen) but like I said, not a fan of the gameplay... Would rather just watch them like a movie.
Oh and about MW3, I strongly agree there. Seeing a character that you played as die... Ye'h...

Halo Reach in general... Especially the final ending cutscene. That's the first one that comes to mind.

Game: Halo 4, when you see that bright light for the first time after you crash land. I also have to give a nod to Red Dead Redemption Undead Nightmare, it was just me over anolyzing everything, but I often felt tense during it.
Movie: Jurassic Park when you see the dinosaurs for the first time, and also seeing the T-Rex, in Imax.
Book: Lord of the Rings, when Gandolf arrives at Helms Deep.
Halo Reach: I hated to what happened to Kat, I was starting to like her. Also I loved how Reach made you feel like you where winning then throws a curve ball your way and it all goes hell after that. I really enjoyed that game.
Halo 4: Yeah you spelled Cortana right .

That moment in the first few minutes of The Last of Us. If you've played it you know what I mean.
Well if you read the book, you totally knew what was going to happen.

In Red Dead Redemption, the part when John finally gets to see his family again...the ride back with the music playing felt like I was riding home to my family
