Then,...
OVERRIDE: Streamline 3.1.esp
AFTER: MyCustomPatch01.esp
This would produce the bug dump above with the warning that MyCustomPatch01.esp was already in the masterlist.
However if I remove the Streamline override and only use the ADD rule for MyCustomPatch01.esp everything works fine, with either revision of the masterlist 1815 or 1816.

Sorry for the edits but I just figured what circumstances this happened under. Still not sure why it happens when I override the order for Streamline.
**EDIT 2** Just found out it depends on how I sort my userlist. If I put the override for Streamline after MyCustomPatch01 then I don't get the error. However if I put it before MyCustomPatch01 I get the error that it's in the masterlist already.
So if I order my Userlist like this...

I still don't completely understand that since the patch isn't added until the ADD command but maybe it's just my fault for ordering it backwards. I mean it makes sense to add the new esp before sorting another esp after it. So yeah maybe this was all just user error with a badly ordered Userlist. :facepalm:
