Broken Steel - Exdending Level Cap

I thought I'd ask this before I actually got to the stage where it kicked in; in case planning ahead is more useful. I realise that BS raises the level cap to 30 and, I'm told, allows you to carry on with the game, but can someone explain how and when this actually happens? I've put it in individual questions to make it easier.

1. Is the level cap raised to 30 upon the installation of BS, or do you need to start playing BS at level 20 to move up?

2. Did the game originally finish at the end of the main quest, whether you'd done all the other stuff or not?

3. And how does this change with BS; can you keep playing indefinately - even once you've finished all the quests - or does it now just stop at 30?

It's raised upon installation. You can start a new game with BS installed, never go near the BS content, and still get to level 30.

Yup, that's right.

No, you can keep playing indefinitely. It replaces the previous, 'hard' ending with one that allows you to keep playing even after the events of the main quest have been completed.
