So I have had this bug in my iOS vault for a couple weeks now but finally taking time out of my daily life to report it.
I have 200 dwellers in my vault and the bug involves two Agility training rooms that become cross referenced in regards to dwellers trying to occupy the two Agility training rooms. It initially happened when I had many caps and decided to rapidly upgrade all my training rooms in succession. One Agility training room on the third upgrade became cross-referenced with another Agility training room.
Both my Agility training rooms are only "one room" wide, so they hold two people each. The bug is that when trying to place dwellers in one Agility training room - they all go to the other Agility training room, leaving one Agility training room always empty, and another with four attempted occupiers even though it is a two-person room.
When the rooms are cross referenced it does not appear that the training actually works, even if there is sometimes a time showing below the dweller, depicting training is in progress. Two dwellers will stand "on top of each other" in the same spot in the room - and other dwellers will eventually just walk off and wander around the Vault, even if showing in the stats as in the Agility training room. Mice / Rats will also show in the cross referenced training rooms even if it is occupied.
One day, I opened the app and noticed four dwellers just randomly disappeared from my vault - I suddenly had 196 dwellers instead of 200. Two of the missing dwellers were Agility trainers - the other two were unrelated and just randomly missing.
I troubleshooted the bug by destroying the two Agility training rooms and rebuilding them - finding that the bug only happens when one of the training rooms is upgraded to the third level. So for resolution, I have the one troublesome Agility training room upgraded only to the second level to avoid the bug.
I noticed that when I destroyed one of the cross-referenced Agility training rooms - that I could still manually place a dweller in the "black void" spot where the room once was. As soon as the dweller is dropped in the former room void, the app crashed.
Here are a couple screenshots on my Dropbox somewhat showing the issue:
