Ok, in the hopes that Beth/Obsidian will take these issues seriously, I'll post my experience here as well.
I bought the game on launch day and started playing straight away. I played every day averaging 5 to 10 hours per day on the game.
I got up to around 45 to 50 hours of game play ... not having experienced a single major problem up to that point ... when I experienced my first freeze/crash. From that point on, the game became unplayable as it crashes randomly in anywhere from 10 minutes to 2 hours of game play. The system completely locks up, preventing access to the XMB or anything else. I have let it set for over an hour in this state hoping it would unfreeze. No joy.
I have a launch date 60gb "fat" PS3 (model CEHCA01) running current (version 3.50) firmware update.
I have downloaded the F:NV patch version 1.01.
PS3 has been kept in a well ventilated shelf, dusted weekly and cleaned with compressed air monthly for four years.
No problems with any other software: have tried multiple Blu-ray games, PSN games, Blu-ray movies, PS2 and PS1 format DVD games and older DVD movies: all work fine.
Have tried: uninstalling game data and reinstalling game
Have tried: turning off all options such as autosave via in game menu
Have tried: playing while disconnected from the PSN.
Have tried: uninstalling/reinstalling while disconnected from PSN without installing the patch.
Have tried: uninstalling/reinstalling while disconnected from PSN and installing the patch.
Have tried: all of the above and starting a completely new character with completely new save data.
Have tried: restoring the PS3 from safe mode.
Have tried: rebuilding the database from safe mode.
Still having massive and completely random freezing and crashing.
I am now, as a last ditch and final effort, reformatting my PS3 just to be absolutely 100% certain that there is no corrupted game data hanging around.
I will then reinstall the game, with the patch, and attempt to create a new character and play.
I can say I was in this same boat, did everything listed here as well to no avail. I even took it a step further and broke down and bought a new slim 360gb PS3 (Since my old PS3 didn't have any problems with other games except Fallout 3 I figured I'd give it to my younger brother). Unfortunately this solved nothing. It can be hours into play or 20 minuets from a cold boot and the game will still freeze up completely and randomly. I figured I would throw this out there because there seemed to be a lot of people stating that their old FAT PS3 keeps freezing making it seem like that in and of it self could be the issue, which it is not.
At one point after recovering from a crash I went about doing other things and finally making a save file only to find out when I go to The Strip it goes from the loading screen to a black screen with sound but never fully loads. Needless to say I had to revert to a save before the crash that probably led to corrupted data and only ended up losing a few hours and a couple levels worth of questing etc.
I decided that I would also mention a couple other issues I have run into after countless hours of playing.
Weathered 10mm - When modded it glitches the gun to the point where it pretty much isn't usable
Dealing with Contreras - From the point of meeting with the man in the Pawn Shop the quest seems to be broken as certain choices which should allow the quest chain to be completed don't work correctly. You can complete the quest but the order in which you do things / and what you say don't match up.
Morales' corpse - When I got to his corpse it was actually buried half way in the ground and attempting to drag his body doesn't work even though it gives the prompt to do so. I also tried with multiple controllers to make sure it wasn't just the button on the first controller.
