Heavy gauntlets include a keyword on them that describes the benefit they will grant the player for unarmed combat (if they have the Fists of Steel perk). Is it possible to create custom keywords that include different values for this? Or is it hard-coded? I can't seem to find how to make custom keywords.

Keywords are in the Object Window under Miscellaneous -> Keywords. They can be created just like any other object
