Can I have my "Born Again" back...

Hey guys.. Today, i just reboot and i got a dog tag name " born again ". Then, when i quit the game and the next time I log in.. Now ! It 's gone :(
[ And yeahh, this might help you guys.. I reboot it in the main menu, not while playing game ]

Can somebody tell me what i have to do...
I really want to get it back : D
