As Magicka in TES lore is fairly diverse and broad i'm sure that changing your gender would be possible. my thoughts are that it would be costly and dangerous though. i think that an artifact with that as an effect would be fairly interesting in Fan Fiction. Hell, Fable II did it.

You could say the same thing about real-life six changes. Why go see a surgeon when you could just learn how to do it yourself on Wikipedia?

Only if something went horribly wrong, enough to completely change your personality and psychological profile.

The point of a six change is to match biological six with the social and psychological gender. :P Having the caster's gender change along with six would put them back in the original position they were in: gender not matching biological six.
