I did not vote because I'm unsure as to which I'd like more, the ability to have disadvantages for some advantages (IIRC like in Daggerfall?) or the absence of such a system. I'm really hoping for a hardcoe mode, I didn't play the hardcoe mode in FONV because I didn't like the Fallout universe in the same manner as I like TES, and so I didn't want extra trouble to give me more entertainment in a game that didn't get me much entertainment in the first place (Excluding VATS and perks, love 'em!).
So, if there is a hardcoe mode, I'd like the option to give myself an advantage at the expense of a disadvantage, i.e. Increased expertise with Fire-based attacks comes at the cost of taking extra Fire-based damage/horrible burn mark somewhere on my face,chest, arms, and/or hands. Velorien's idea of a "background option" would be nice, but it would feel like birthsigns if you had to choose (On that note, are birthsigns even going to be in the game?). Even if there wasn't a hardcoe mode, I'd still like if the option existed. The possibly to gain new and special perks (Or "feats, abilities, etc.", if you dislike the word "perks"), increased starting stats, more starting money, etc. at the expense of some disadvantage during Character Creation would be a welcome addition, in my opinion.
