I just finished the game on "Super Soldier" difficulty, I only received the achievement for "Veteran." I went through the 'replay a mission' menu and it said "Completed under Super Soldier" for all of them.

How can I get the achievement to be unlocked, or am I out of an achievement?

Try replaying the sub mission on SuperSoldier by starting an entire new campaign, not by selecting the mission, and if that doesn't work, then try replaying the last mission..
