In MW, the difficulty is largely controlled by what you attempt. It's less "levelled" than OB, and not "scaled" at all, so you will face both things that are levelled to you, and things that aren't. If you "push" the character and go places where a "fresh meat" character doesn't belong, you will sometimes get your posterior handed to you on a platter. If you "play it safe", you can wait as long as you like until you feel "ready" to take on the tougher challenges.
The slider merely controls the ratio of damage done versus damage taken. It has no effect on spawns, rewards, or quests.
I use the "best attack" option. It saves a lot of awkward mouse movement, and why would you "slash" with a spear?
