It's a latency thing. On your screen you are shooting a guy in the head or torso etc. but the player's real position on the map isn't 100% where you see it. The host player does not have to deal with this issue unless the game has some kind of lag compensation code in attempt to even things out. This is off topic, but they say this is exaggerated in Black Ops and that's why a lot of players complain about having lag when they are the host.
