This problem is affecting me a little differently now... I'll start with your questions.
1) I believe the account that I was using to login to the game with was my old Crysis 1 login information, but it wasn't linked with Gamespy anymore.

3)The website account I created with the same nickname as the in-game account was created with Chrome.
4)The website account was my first gamesas.com account (to my knowledge).
5) On the weekend I deleted my website account, and initiated a password reset for the e-mail address that was associated with the information I was using to login to the game with. I went through the process, and was then able to login to the website for the first time with the account I was using in the game (the website account I deleted and the in-game account both had the same nickname). Edit: updating the password also affected the game login for the first time.
My username was now showing up in the upper right as this old e-mail address. When I clicked the little picture next to the name, it took me to the account screen, but updated my username to be my nickname (what you see now attached to this post). It also then associated all of my old messages and posts from the web account I deleted with the in-game account.
I can no longer login to the website using the drop down menu in the upper right. I have to go to http://www.gamesas.com/user and use the old e-mail address (not the nickname... it doesn't work) to login to the site. Once I do, it still initially shows my username as the e-mail address. Even more strange... if I try to initiate a password reset, it does not go to that old e-mail address anymore, but it goes to the e-mail address that was associated with the web account I deleted. Clicking the links that come in those e-mails does not work.
I'm sorry if that is confusing... but... imagine how *I* feel about it.

**UPDATE**
So after looking some more into my current situation, there still seems to be a disconnect between the account that is on the site, and the account that is in the game. If I change my password on the site here, it does not update the password attached to the login I use in the game. I can only change my password if I request another password reset... which seemed to work properly this time.
