I'm having two errors. Finally got more weapons, armed my staff, they were able to take out Raiders without out, all of a sudden radroach attack and killed half the population, how they go from easy to kill to, they kill half my vault.

Also the biggest one now, sent out explorers, one found two outfits and one found a weapon and an outfit. They come back and I collect the items, and they are not in my vault, I can't use them.
