When you choose who to go into the keep with in the begining scenes, does it change the game that much? Is it possible to change factions or do a different quest line?

Nah, it doesn't change it that much - it just means you talk to somebody different in Riverwood, which changes absolutely nothing.

Not really, apart from who you kill in the beginning.
