There are a lot of good ideas from Van Buren. I'd like to expand on my Hounds of Hecate proposal. I don't know if this would be a major faction, most likely not but still an important one. In terms of importance in the game, I'd compare them to Great Khans, not the Legion or NCR. There would be one town controlled by them, ,maybe a few other encampments, and several quests.
Not sure what they'd be called exactly, Hounds of Hecate sounds best, I don't like Daughters of Hecate which is the other name on the wiki. The wiki says their capital city is Ouroboros. They are supposed to be in the American Southwest, and New Vegas did have them as the "Vipers" but clearly there was nothing else to them to show that they were the real Hounds of Hecate, meaning you could still put them in another region and not have any conflict.
They are ruled by a woman named Hecate who sees herself as a goddess (or a living human incarnation of a god) and demands worship. It is very similar to the Legion but it avoids problems I had with the Legion. NV made the Legion too evil for my liking. You have to be an evil character to look past the slavery and crucifixion and all that and join them.
Under Hecate (pronounced He-KAH-tey)'s rule, there is no slavery or brutal stuff and they don't act like the Raiders or Fiends. All the more reason why people revere her as a benevolent god. But still Hecate is....crazy. She is an egomaniac, so she is kind of evil in that regard. and so her followers are crazy too.
They're very pagan, like witches...yes, they believe in magic (read on before you bash me), but it's obvious to the player that Hecate's magic is not real, mere illusion. So this wouldn't be putting magic in the game, it would be introducing beliefs about magic. Primitivist also, this is my updated version of the "Ancients" faction I thought of earlier, people who are against technology. Hecate warriors don't use energy weapons or guns. They use melee weapons and projectiles.
Instead of dealing in chems, they make a lot of alcohol, especially wine.
If you want to make fun of the fact I compare them to witches (pagans who believe in witchcraft, not stereotypical witches who ride broomsticks), remember if the next game takes place in the Commonwealth (Massachusetts...) it would totally fit because of the Salem witch trials. I think it also fits with the post-apocalyptic nature of the game , like the Children of the Atom cult in Megaton. Traditional religion has collapsed, there are new weird ones, even Saint's Monica Church in Rivet City which is similar to Christianity, has nothing to do with Christianity. and there's also the Bright Brotherhood in New Vegas. I like crazy cults in Fallout, there should be one in Fallout 4 too.
I think that if there were to be a really "evil" aspect it might be human sacrifice. Maybe these people aren't forced....they might have a lottery type of thing, and their faith is so strong they accept what they have to do, or they even willingly offer themselves. One quest could revolve around trying to convince somebody not to sacrifice themselves in the name of a cult.
