Don't post a dxdiag report, if you aren't going to post the whole thing (in spoiler tags). However, my guess is that your issue is due to the discrete graphics chipset on being active for Fallout 4.

Did the game ever work?

@OP The CPU is technically below the requirements, but I doubt it would prevent it from loading. Has anything changed since the last time it loaded correctly. New hardware, new applications, etc...?

I don't believe a 2.7 CPU is the reason for a CTD upon startup, especially if the game ran fine earlier. I have a 10 year old CPU, it's not i7, i5 or even i3 and the game runs fine, though not at optimum graphics levels. I also have only 4GB DDR2 memory. I do have a gtx 970 though.

When I read you reinstalled WIndows and you still had the issue, my jaw dropped. It could be your memory kit, maybe. Just for kicks, kill the OC / XMP, and run your kit at it's native speed. What OS are you using? If you're using 64 bit windows, maybe try 32. If using 32, try 64. But try no OC first. If you are running native, try underclocking, you know, just to see.

That's about the only thing I can think of, that Bethesda coded in a [censored] hardware check and that prevents it from running. I can run it at 1080p 60fps fine (except in cities but hey) on high, so it should run fine like it did for the past 25 hours.

I'm using Windows 8.1 64 bit, just like I was using the first 25 hours. I'll try dropping the XMP profile and see what happens, even though It has perfect stability in any other game except FO4.

Try copying your save file folder to another directory and see if it hangs. If it says there was an error with one of the save files, click skip then go and delete that particular save file. After deleting the save file that was throwing the error, my game no longer freezes at the "Please Stand By" screen on start-up.

So I tried dropping the XMP profile, but that didn't fix it.

Also, I removed all the save-files since I reinstalled windows, that didn't solve it either...

oh my god yes I fixed it guys!

Turns out it was a corrupted savefile. Even after reinstalling windows, Steam just re-downloaded all my saves from the cloud.

So I removed all my savegames except the autosaves and the quicksaves and now it works again!!! :D

yeah well got a response from Bethesda, they say the PC is more than capable(they have the full dxDiag) and not sure why... pointed me to MS for a solution ...

The CPU comment was for the OP. Your computer should run the game, but 'switchable' graphics has major flaws, where it doesn't work correctly.

If you get your computer to boot again, the following registry key edit might allow the discrete card to engage properly
