Seeing as the new patch arrived, which should have fixed most of these bugs (but it seems it didn't), I'll start crossing out bugs that have been fixed (

Game Breaking Bugs
Not really game breaking, but this can't be stressed enough: the game constantly locking up and crashing is absolutely terrible. This is extremely prevalent while fast travelling or entering zones/buildings
"New Game" glitch: xp going over 100% but not allowing player to level up.
Saves may corrupt or just don't want to load anymore (the roulette wheel keeps spinning forever)
Various stages mid game - DLC error that corrupts most if not all saves. (dlc error work around below)
Trying to Continue from a DLC corrupted save causes system lockup vs loading a saved game.
Controls randomly locking up not allowing progression.
During "For The Republic, part 2" and/or "You'll Know When it Happens", you might get stuck inside the Hoover Dam Visitor's Center - all the doors leading outside will say they require a non-existent key.
The Strip becoming locked requiring a non-existent key.
The Strip black screening upon loading, which also corrupts auto-save.
The game can suddenly become unable to detect the storage device.
Becoming Stuck in the Lucky 38 Penthouse because Victor wont respond so travel not available
Location Bugs
Location "Lucky 38": All items that have been placed in the Craft Lockers in the Presidential Suite for savekeeping disappear upon leaving the suite.
Location "Goodsprings": Some players are encountering a permanent fps drop after leaving Goodsprings.
Various bugs surrounding Novac and the Helios One Power Plant. Sometimes when you leave Helios One with the back door Rey, Colmillo, Basura and 2 Molerats will fall out of the air and will become hostile to the player. Old Lady Gibson might also fall out of the sky with the dogs and molerat attacking the player. If you get near the NCR troopers while the dogs and Old Lady Gibson are attacking you, they will also become Hostile.
Location "Helios One": Hostile NCR guard dogs can be found outside the main building near the solar arrays, even when you're friendly with the NCR, and attacking them will cause all NCR troopers to become hostile as well.
Location "Deathclaw Promentary": The lootable bodies and/or the deathclaws might not spawn.
Location "NCRCF": Killing all the Powder Gangers in the NCRCF compound might cause the game to crash upon travelling (see http://www.gamesas.com/index.php?/topic/1153912-fallout-new-vegas-bugglitch-list-v3/page__view__findpost__p__17348671)
Location "NCRCF": The guards at the NCRCF might become hostile after taking care of the south camp even after paying off the gate guard (see http://www.gamesas.com/index.php?/topic/1153912-fallout-new-vegas-bugglitch-list-v3/page__view__findpost__p__17348635)
Location "Vault 11" Vault 11 turrets are wrongfully a part of the Lucky 38 faction - if you destroy them before getting to the strip, you never get a chance to talk to Mr H. as the casino house robots attack as soon as you enter.
Location "REPCONN test site": Entering the test site might cause the game to lock up

Location "The Fort": A lot of people are experiencing crashes and lock-ups when (fast) travelling to and away from Fortification Hill. People have also been getting stuck in the Weather Station.
Location "188 Trading Post": The game crashes consistently for some people when trying to reach the trading post
Location "Lake Mead": The is a crevice in the lake which you can enter without problem, but you get stuck when trying to leave (see http://www.gamesas.com/index.php?/topic/1153912-fallout-new-vegas-bugglitch-list-v3/page__view__findpost__p__17733673)
Location "Camp Guardian": A large low detailed 2D texture can be found near the Camp Guardian Cave (see http://www.gamesas.com/index.php?/topic/1153912-fallout-new-vegas-bugglitch-list-v3/page__view__findpost__p__17733673)
An issue in the train tunnels between Raul's Shack and Brotherhood of Steel Safehouse and south of Nellis Air Force Base. Going up stairs that lead up to a small room inside the tunnel causes players to fall through the ground consistently.
Location "Sloan": Chomps Lewis might randomly get killed by something unknown (when fast travelling), causing villified reputation with the NCR.
Location "Camp Guardian": Entering Camp Guardian might cause your screen to become blurry until you reload.
Location "Lucky 38": If you kill Mr House before talking to him, you lose the ability to access the cocktail lounge, making it impossible to find the snowglobe there.
Location "Vault 03": When stopped by the Fiend Guard (upon entering) a player didn't have the required Speech Skill level to be treated as a friend so he backed out of the conversation and the Vault. Now, on "Aba Daba Honeymoon", he can't progress as he no longer even have the option to initiate dialogue with the Fiend Guard, so he can't get into the Vault without them becoming hostile. Wearing Great Khans Faction Armour does NOT alleviate/resolve the problem.
Location "Vault 34" Ghouls can sometimes travel through doors and walls.
Location "Vault 34" Saving your game in the Overseer's Office or the Armory will corrupt your saves.
Location "Vault 11" Entering Vault 11 when trying to recover parts for the Broterhood air filtration system might cause lock-ups.
Location "REPCONN Headquarters L1" shop store room - two safes overlaid on top of one another.
Location "BoS Base LvL 1" Medical Case cannot be opened in the Medical room.
Location "BoS base L2" repair room Damged armor/weapons cannot be picked up.
Location "NCRCF": Running along the outside of the NCR correction facility, you will find outhouses INSIDE the facility you can select and teleport to, effectively bypassing the gate.
Location between "NCRCF & P.G. Camp South" Powder charges floating above ground.
Location "The Thorn" doesn't declare you the winner at the end of rounds and wont open any of the doors/gates leading out of the arena.
Location "The Thorn" Creatures becoming stuck behind doors. Talking to Red Lucy will not fix. Leaving and re-entering the arena does not fix it.
Location "The Thorn" Creatures wont attack each other.
Location "The Thorn" If you fall in the pit the side gate is not open so you can't leave. The other doors you/the creatures normally come out of are also locked.
Location "The Thorn" Guards will attack you while you are in or start a fight.
Location "The Thorn" Using explosives in the arena causes guards to attack you.
Location "The Thorn" invisible wall covering 3/4 of entrance door to arena, can still bypass however.
Location "McLearan Airport" - torturing the legion soldier while having companions causes them to kill him and player becomes locked inside the room.
Location "Mirror's Field" causing various problems including game freezing, npc bugs, music bugs, random item spawns, etc.
Location "Silver Rush": When the player enters the Silver Rush for the first time, the scripted event involving the executed prisoner might not play out properly. Dying when first entering the Silver Rush might aggravate the issue.
Location "Silver Rush": When taking some companions with you when you enter the Silver Rush for the first time, all guards may become hostile.
Location "Silver Rush": Gloria Van Graff's inventory does not level with the player.
Location "Atomic Wrangler Casino": After recruiting Fisto as the sixbot and completing the quest, if you start speaking with him and then saying you are not interested, after every time you exit the speech, you receive 10 caps. Can be done an unlimited time.
Location "Atomic Wrangler Casino": Despite Atomic Wrangler's reputation as a free-swinging brothel and dialogue options with Francine Garret that would suggest the same, there seem to be no prosttutes in the casino beyond ones the player recruits in the Wang Dang Atomic Tango quest (this might not be a bug, but if it isn't, it's a terrible design choice).
Location "Quarry Junction": Climbing the crane on the eastern part causes you to fall into it and get stuck.
Location "Nipton": Some people are experiencing crashes when fast travelling to Nipton or entering it by foot.
Location "Mojave Outpost": Crashes after visiting Nipton.
Various Locations along mountains, cliff sides the player can fall through and become stuck.
Visual / Audio Bug(s)
Camera issue involving first person perspective where the player's arms go up and down on the player screen... almost looks like your character is rope skipping (this hasn't been fully fixed, but it seems like it's not as bad as before)
Audio glitch involving ambient sounds. Several people have reported that the ambient sound of the previous location gets carried with them when entering a new location, e.g. hearing crickets when entering a house from the wasteland.
Some people are experiencing issues with NPC pop-in after some hours in the game. Enemies only appear visually when you've gotten very close by, long after they've appeared on the radar, even without EDE and with average perception.
Issue involving dead bodies dropping down from the sky after (re)loading an area. Can be done consistently in the Sheriff's Shack in Primm, you can hear the bodies fall.
Issues with the ingame radio stations, including a volume glitch which causes songs to be whispered while the announcer speaks at normal volume and a glitched playlist with some songs being repeated over and over, while others are never played
An issue concerning miniguns/gatling lasers: using them outsite of VATS, the firing sound will continue even after you've stopped firing and the firing sound is out of synch with the actual firing.

Drawing a pistol or any other sidearm will cause the "Draw Audio Sound" to double up. (see http://www.gamesas.com/index.php?/topic/1153912-fallout-new-vegas-bugglitch-list-v3/page__view__findpost__p__17412435)
Weird audio glitches, including hearing doors and giant gates opening and closing when walking in the wasteland or inside zones like freeside with no doors closeby.
The hacking and lockpicking interface might glitch, causing hacking to show a blurry unfocused screen and lockpicking shows a blank computer monitor instead of the usual lock.
Blank Pip-Boy (Black) screen issues when using computer terminals, lock picking, sniper rifle scoping, binoculars, slot machines.
Male characters using female voice when recieving dmg and vice versa (might be connected to a glitch that causes the game to think you've switched genders, see Game Functionality Bugs).
Pip-boy light glitching so player cant see Pip-screen when brought up, Pip-boy screen might glow bright green
Questline Bug(s)
Quest "I Put A Spell On You": Multiple quest bugs including the train exploding even after you have diffused the bomb, the bomb not being on the train, the train leaving before you can get on it and not being able to report to the colonel.
Quest "The White Wash": You cannot start this quest if you kill Silus because Boyde will not want to speak with you.
Quest "You Can Depend on Me": If you have completed "There Stands the Grass" quest, Dr. Hildern might glitch and not accept your parcel, making the quest uncompletable.
Quest "For The Republic, part 2": Completing "You'll Know When It Happens" first makes this quest uncompletable.
Quest "For The Republic, part 2": After being briefed by the ranger, the game might crash
Quest "For The Republic, part 2": If you talk to the Omerta Receptionist before speaking with the woman at the NCR embassy, the quest become impossible to complete
Quest "Caesar's Favor/Caesar's Hire": Fast travelling away while the quest giver (Disguised Frumentarius) is approaching permanently disabses these quests. Additionally, completing Caesar's Hire makes it impossible to get Caesar's Favor later on.
Quest "Veni, Vidi, Vici": It's possible to get permanently stuck in the Legate's tent instead of teleporting to Hoover Dam (see also: http://fallout.wikia.com/wiki/Veni,_Vidi,_Vici)
Quest "Wheel of Fortune": Boxcars won't initiate dialogue about radiation suits after "Booted" is completed and/or after giving him (the maximum amount of) Med-X, making this quest uncompletable.
Quest "Wheel of Fortune": Sometimes Logan will keep telling you that there have to be more NCR parts in the police station even though you collected everything.
Quest "Wheel of Fortune": Exploring the Hidden Supply Cave just outside Nipton prior to this quest can cause the dialog option relating to it to become unavailable when speaking to Boxcars, thus preventing the quest from progressing.
Quest "Come Fly With Me": After getting rid of Harland, talking to Davison might randomly make him aggresive.
Quest "Come Fly With Me": After completing the quest by sabotaging the rockets, one ghoul still stands on the launch pad.
Quest "Come Fly With Me": After completing the quest by having Chris Haversam sabotage the rockets, when you return he seems to have amnesia and runs away from you scared.
Quest "Come Fly With Me": After completing the quest, the nightkin attacking the McBride's Brahmin might become glitched.
Quest "Come Fly With Me": Completing the quest might cause the entire game to glitch
Quest "My Kind of Town": Completion can result in everyone being disgusted by the occupation of Primm by the NCR, even when you convinced Meyers to be the sheriff.
Quest "My Kind of Town": Meyers can turn hostile automatically at the NCRCF even when you don't accidently hit him. This shouldn't happen since he isn't a powder ganger.
Quest "My Kind of Town": Primm Slim cannot be recruited as the sheriff if you already found Vance's 9mm sub-machinegun.
Quest "I Fought the Law": Upon completion, Radio New Vegas always reports that the raid on the NCRCF by the NCR has failed, even when the raid was succesful. Random NPC's will also talk about the raid having failed.
Quest "I Fought the Law": The ending cinematic mentions that the NCRCF was never retaken by the NCR, even when it was.
Quest "I Fought the Law": When assisting the NCR, the NCR troops will rush the administration building, ignoring the cell blocks and visitor center. In turn, the Powder Gangers in the cell blocks and the visitor center will not exit during the assault, requiring the player to manually clear them out. This may be the cause of the above two bugs, as not killing all of the gangers may make the game detect the assault as a failure. This may also be connected to Meyers glitching out.
Quest "You'll Know When It Happens": Saving President Kimball might result in your reputation with the NCR and Ceasar's Legion to reset to neutral.
Quest "You'll Know When It Happens": After Kimball's speech he may just stand there, never going back to his vertibird.
Quest "You'll Know When It Happens": When given the mission to protect President Kimball from assassination and you have hacked the terminal and checked the blood stain, you might not be given the option to talk to the NCR Ranger.
Quest "You'll Know When It Happens": Some people are reporting that Kimball cannot be saved due to a bug.
Quest "Return to Sender": Leaving Camp Golf after confronting Chief Hanlon might result in crashes.
Quest "Aba Daba Honeymoon" problems completing due to Motor Runners helmet issue.
Quest "One For My Baby" Boone not recognizing the Bill of Sale as proof of guilt.
Quest "One For My Baby" If you steal from the safe in the Dino Dee Lite lobby before speaking with No-Bark, you do not have the option to take Jeannie May Crawford out in front of the dinosaur. Also, you might not have the option to take Jeannie out with you in front of the dinosaur, period, no matter what you've done.
Quest "One For My Baby" Binding Boone's Beret to a directional pad key may result in a crash.
Quest "One For My Baby" Speaking to Boone during the quest, as he's heading to the sniper post, will cause him to become stuck. This makes it impossible to complete the quest.
Quest "Ghost Town Gunfight" may be causing some vendors to become unable to buy/sell from
Quest "The House Always Wins I" Mr. House retaining the Platinum Chip without acknowledging he's been given said item.
Quest "How Little We Know" Cachino doesn't start the attack on the two bosses and becomes locked in a loop
Quest "Restoring Hope" attempting to enter Dead Sea's Hut can cause game to freeze
Quest "Still in the Dark" Harkin/McNamara bug near the end of the quest making it unable to complete
Quest "ED-E My Love" Quest Lockup involving the Brotherhood of Steel asking you to see them (see Companion Bugs)
Quest "ED-E My Love" The first part of the quest directs you to the Gibson Scrapyard, but if you've spoken to Old Lady Gibson before, nothing actually happens there.
Quest "ED-E My Love" Completing the quest doesn't remove it from your questlog.
Quest "ED-E My Love" Upgrading ED-E results in the loss of everything in its inventory.
Quest "The House Always Wins Part 1": Swank doesn't always recognize the Chairman Job Offer Note.
Quest "The House Always Wins Part 2"": Some people can't upload the data from the chip to computer in the army bunker.
Quest "For The Republic, part 2" You might get stuck inside the Hoover Dam Visitor's Center - all the doors leading outside will say they require a non-existent key.
Quest "You'll Know When it Happens" You might get stuck inside the Hoover Dam Visitor's Center - all the doors leading outside will say they require a non-existent key.
Quest "You'll Know When it Happens" Completing "For The Republic, part 2" first makes this quest uncompletable.
Quest "Old School Ghoul" Loyal might disappear in Nellis Air Force Base after completing "Volare!", causing this quest to become uncompletable.
Quest "For Auld Lang Syne" Arcade Gannon might stop giving the dialog option asking him to follow the player, making this quest uncompletable. This seems to be connected to Arcade being recruitable when you already have another human companion.
Quest "For Auld Lang Syne" After completing the quest, Arcade Gannon will ask whether he should stay with the Enclave Remnants or return to the Followers of the Apocalypse, but the player will be unable to answer. Afterwards, Gannon is completely glitched.
Quest "Birds of a Feather" If you choose to guard the door with Simon there is a potential bug. After 4 npcs, there will be a special NPC who will chat with Simon. If you aim your weapon at him he will stop talking to Simon but permanently stand outside of the Silver Rush. If you enter the Silver Rush, you can take the next quest to deliver a package, but once you leave the outside post, Simon will attack you thinking you stole Van Graff weapons and and armor.
Quest "Birds of a Feather" If the Courier denies the fourth customer entry, he will turn hostile. If the Courier deals the finishing blow in the resulting fight, Simon may freeze in place.
Quest "Birds of a Feather" Guarding the entrance to the Silver Rush at one point you need to move a body out of the view of customers.Trying this by dragging the body to the right of the building never works. If you go too far, Simon will accuse you of stealing the armor and attack you.
Quest "Birds of a Feather" Pickpocketing Jean Baptiste might cause him to unequip his weapon, glitching out the sequence in which he should shoot Cass.
Quest "Crazy, Crazy, Crazy" After resolving this quest by repairing Rhonda, Tabitha will become friendly. However, upon returning to Black Mountain, Tabitha and her super mutants may be hostile again. This may be related to Rhonda randomly dying/disappearing.
Quest "Dealing with Contreras': Completing the quest by helping Contreras through the use of Speech doesn't earn you This Machine as a reward. If at a later point in the game you sell out Contreras to Lt. BoydI, she will give you This Machine, which will consistently corrupt all saves made after that point.
Quest "Boulder City Showdown": Some people report that choosing the diplomatic route still causes the Khans to become hostile.
Quest "Wild Card: Change in Management": Trying to complete this quest after "The House Always Wins, I" causes this quest to glitch.
Quest "Still in the Dark": Hardin might never take over the Brotherhood of Steel when he should.
Quest "Still in the Dark": Various bugs that may result from following Hardin's and McNamara's questline at the same time.
The unmarked quest from Captain Parker involving Keith: When accompanying Parker to arrest Keith, Parker might glitch upon entering the building Keith's in. He suddenly teleports to the room with the marked cards and never arrests Keith.
The unmarked quest from Captain Parker involving Keith: Passing Keith's barter check while Captain Parker is on his way to arrest him resets the quest (Captain Parker keeps giving you the instructions) and makes it impossible to complete.
Quest "My Kind of Town": Meyers can't be recruited anymore after completion of "I Fought the Law", he gets stuck in a dialogue loop.
Quest "Oh My Papa": There is no way of completing this quest if you've spoken to Melissa and did the "Don't Make a Beggar of Me" quest.
Quest "Old School Ghoul" Taking to Ranger Andy might not register with Raoul, causing this quest to become uncompletable.
Quest "That Lucky Old Sun" Infinite XP involving Ignacio Rivas and overloading Helios One
Quest "Come Fly With Me": After you convince Chris Haversam he's human and tell him not to take revenge and to move to Novac, he might randomly keep standing at the underground launch pad after completion of the quest, stuck in a dialogue loop.
NPC Bug(s)
Hardin might never take over the Brotherhood of Steel.
Buying and selling from Mick and Ralph in Freeside is impossible
Julie Farkas stopped acknowledging the fact you helped her out with her chem supplies
Various bugs associated with Boxcars (also see: Wheel of Fortune bugs)
Trying to buy from travelling merchants might cause lock-ups.
Various other bugs regarding travelling merchants, including them locking up during their patrols, becoming stuck in one place, and them not updating their inventory.
Ada Strauss might disappear from Novac.
Old Lady Gibson might disappear (might be connected to her falling from the sky near Helios One).
Some players report that being villified by a faction might not cause that same faction to be hostile. Other players report that being friendly or neutral with some factions, still causes them to attack you.
At the Legion Raid Camp south of Novac if you attack the Legion stationed there the tied up Powder Gangers can escape freely as if unbound already.
NPC "Red Lucy" face will lock up into a strange animation and she will appear to grit her teeth and snarl at the player.
Items cannot be sold to the same faction they're stolen from (dont appear in the vendor list)
Chris near the end of Come Fly With Me becomes stuck in a loop saying he's waiting for the launch even after you've completed the quest
Multiple No-Name npcs saying the opposite of what happens involving multiple quests - like saying Primm accepted NCR rule even if you get the NCRCF sheriff.
Buying/Selling duplicate items to an npc causes semi-infinite money gain.
Infinite XP Bug involving Mr. House Medicine 35+ and repeatedly stating his life span is impossible.
Blake the Crimson Caravan Merchant can get stuck inside one of the barracks, making him unable to return to the outside of his shack.
NCR Ranger companion, added to your crew during "No, Not Much", turns hostile toward securitrons at The Strip gate unprovoked.
Sometimes NPC's spawn 3 or 4 times at the same location, causing some kind of fused, glitched mess, which can't be shot at or talked to and you can just walk through them. This happened to me with Jackals (the raiders) and NCR troopers.
During idle chat, some NPC's talk with the wrong voices (children as advlts, women as male,...).
Doc Mitchell's head and animation might be completely screwed up during the opening sequence (see this).
Trying to trade with Daniel Contreras is impossible for some players.
Enemy NPC Bug(s)
Bad spawning points and/or pathing problems involving enemies being stuck in the ground. A clear example of this are the Giant Ants running back and forth between their Ant Mound and the Giant Radscorpion corpse that's lying on the road between Nipton and Mojave Outpost (well, okay, they don't even reach the Radscorpion corpse properly, but it's clear what Obsidian was trying to accomplish). Just stand there and watch for a little while. They all start to run while their bodies are vertical and some bodyparts, mostly their head, gets stuck in the ground. This may even lead them to bug out completely. Just walk around the Ivanpah Dry Lake and Race Track, you'll find a lot of motionless ants that don't seem to be truly dead.
Enemy NPCs spawning below the ground or spawning in some sort of semi-death state (they don't move, but you don't get the regular "A) Search ..." command. Pressing A still loots them though). This happens a lot with bark scorpions near Ivanpah Dry Lake.
Powder Gangers that spawn when exploring the wasteland and Powder Gangers at some Powder Ganger Camps are hostile even when you're neutral or friendly with the Powder Gangers.
After becoming villified, some people are seeing an enormous amount of Legion Assassin spawns while others see none at all.
Possible Nightkin bug involving cloaking and loading a save (phasing cloak might be intentional however so not making this official yet).
HUD & Game Functionality Glitches
V.A.T.S. not firing and locking player in "VATS Mode" with everything moving in slowmotion until player or enemies are slain
The game always thinks it's sunday upon loading a game, which messes up merchant inventories
Crashes when pressing "Continue" in the main menu when starting the game
Performing the Ranger Takedown might cause the game to crash
Killing some creatures, like Feral Ghouls, sometimes gives you good karma, especially when you're sneaking (if not a bug, this is a strange design choice, to say the least).
Quick Draw causes pistols to holster incorrectly, making it take twice as long to draw them again. Essentially, this renders the perk useless for pistols.
The Ninja Perk is broken as it doesn't add a flat 15% critical strike chance.
The Stonewall perk is broken, it will only give you +5 DT when you are using melee or unarmed weapons, instead of when you're attacke by them, as it's supposed to.
The Super Slam! Perk does not work with unarmed weapons from the DLC
Players are experiencing a lot more crashes than Fallout 3 and some people aren't able to start the game at all, they just get a black screen.
Extremely long loading times when compared to Fallout 3, even after installation.
Extended play causing game to slow, lag and freeze (possibly due to heat).
Ash Piles can't be looted sometimes.
Some perks, including Grim Reaper's Sprint and Ninja, are glitched or not working right.
Some are having issues using computer terminals: they only hear the typing sounds when using one, but the computer terminal interface doesn't show up.
V.A.T.S. target appearing off camera if player is hit prior / during loading of vats.
V.A.T.S. sometimes shows the enemy's health as zero when he clearly isn't dead
Your characters gender gets mixed up by the game: for example, while you still look like a man, every NPC thinks you're a woman and you sound like a woman as well when getting hit (see Visual/Audio Bugs)
Controls randomly locking up not allowing progression (also listed under game breaking)
Some people can't use the wait command to wait for a few hours because the game keeps telling them that they "can't wait in this location". This happens everywhere.
Changing weapons with the d-pad often doesn't work.
Dying while throwing dynamite/shooting a gun in VATS and then reloading a previous save, cause you to throw dynamite/shoot automatically upon reloading.
Reputation with factions might reset without warning and without any real cause.
Multiple aiming bugs, including players aiming sideways in third person mode and the crosshair not lining up with the weapon when aiming up or down.
Items/Weapon Bug(s)
Several bugs concerning the minigun: The firing sound playing out of synch while firing and continuing after you've stopped shooting, the ammunition backpack becoming stuck to the player even after switching out weapons the weapon remaining in the player's hand in third person view even after holstering it.
Unkown if this happens elsewhere, but when taking the items in the NCR Safehouse (NCR Ranger Combat Armour, NCR Ranger Helmet, NCR Salvaged Power Armour, NCR Salvaged Power Helmet, Trail Carbine, Assault Carbine, Hunting Revolver, NCR Ranger Patrol Armour), the items weight is applied, but they do not appear in the Pip-Boy interface. Leaving the Pip-Boy and re entering seems to make the items appear in the Pip-Boy.
Gatling Laser/Grenade Machinegun appear on the PC's wrist when taking off/equipping Power Armour (Remnants and Gannon Family Tesla Armour Included) and NCR Ranger Combat Armour, cannot confirm other armours as this is the only armour I've noticed it with. Cycling through your hot key'd items once or twice seems to reset it.
Quest items (Van Graff's Armor, Motor-Runner's Helmet, Rocket Souvenirs, Rebreather,...) become locked in the players inventory even after correlating quest is completed.
Repairing faction armor with non-faction armor through jury-rigging might attach the faction affiliation to your character permanently, even after removing the faction armor.
The Grimy Pre-war Businesswear uses the same texture as the dirty one when equipped by a male character. Additionally, when females wear the Dirty Businesswear, it uses the same texture as Kimball's suit
NCR ranger combat armor: Most rifles go through the trench coat when holstered.
If you're wearing a metal armor while turning it into a gecko-backed metal armor, it hardcodes the -1 AGL loss of the metal armor onto your character
Wearing any prewar clothes that give an agility boost and returning them using the vending machine in the abandoned BoS bunker hardcodes the +1 AGL gain of the prewar clothes onto your character
Aiming with True Iron Sights with two-handed weapons isn't accurate, they shoot slighty to the right of where they should.
Euclid's C-Finder not firing properly (may cause save corruption) also leaves tracking lazer on screen.
If you completed "That Lucky Old Sun" before obtaining Euclid's C-Finder, you will not be able to activate it or use it. It will always show that you have no charge and just makes a clicking sound when trying to use it.
Repeater Shotgun locking up / trouble reloading / shooting stuttering.
Glitch involving guns, mainly lever action guns, being unable to fire for a few seconds after reloading has been completed.
12 Gauge Bean Bags are not working as intended, they cause a huge amount of damage but don't knock out NPCs.
The .44 magnum handgun with HD cylinder and scope mods cannot be repaired with a repair kit. Using one DECREASES the condition.
Bugged weapon animations when firing when no ammo is in the gun.
(pre-order item) Weathered 10mm + Silencer Mod Visual & VATS problems (pull up the gun and only shows your hands holding nothing, and screen turns shade of white)
Sometimes in the middle of combat you will find that you cannot fire your weapon, aim down its sights or go into vats but you are able to do everything else. This lasts for a few seconds and then everything is ok again. It's not a framerate issue.
Adding a lot of mods to a lot of different guns in one PipBoy session might cause the game to crash
NCR ranger helmet: It's value is 0, and it cannot be repaired by vendors.
Carrying a lot of Catseye might cause it become activated when companions are near you.
Gettting implants might cause lockups and save file corruption.
Items "hiding" behind others in the inventory, for example fully repaired varmint rifles "hiding" behind a modded varmint rifle. Only the modded varmint rifle shows up in the inventory until you drop it. This also happens with fully repaired items. Fully repaired items are not showing up with a count in inventory but the invisible copies are using up carrying capacity.
Bought Caravan cards are not being added to the Caravan Deck, they stay in the inventory separately. This means they aren't available when playing Caravan either.
After getting the Rock It Launcher Scehematic it cannot be crafted at a workbench because it does not appear in the list.
Ant Nectar addiction unable to be healed through "fixer" or wasteland doctors.
Companion Bugs
Companions will not zone into the northern-most section of The Strip (from south Freeside).
Players are unable to ask followers to wait at Lucky 38 for all but the Yes Man main quests line.
Having Raul in your group while learning the Ranger Takedown from Ranger Andy in Novac causes the game to crash.

ED-E delivers a message from the Brotherhood of Steel after enough trigger words are activated. During the last paragraph of this speech, the speaker's dialect changes to that of a hispanic male, and the player is consistantly stuck without being able to bring up the Pip-boy or move.

Trying to heal a companion with a stimpack using the radial interface might kill them instead when they're poisoned.

There is no way to cure poisoned companions.

Being in Nipton with Veronica should trigger dialogue according to the official guide, but it doesn't.

Companions can lose their weapons and other equipment randomly, most noticably Boone and his scoped hunting rifle.
Companions not adhering to the "Do Not Attack Until I Do" command.
After telling companions to wait, they sometimes appear next to you after a loading screen anyway.
When a companion gets killed, the game might still think that companion's with you and you won't be able to recruit a new companion.

Dropping the cards required for Dead Money's "Dead Man's Hand" challenge, and picking it up, adds to the complettion of the challenge.
