The Geforce 9400 GT is the same card as the Geforce 8500 GT, and therefore actually a bit BELOW the minimum Fallout 3 Geforce (you should ignore the foolish business from Obsidian regarding Fallout NV minimum video graphics. It's wrong).

The 7900 GS was a somewhat better card than the minimum for Fallout 3, unlike yours. It is the "hundreds" number that makes a difference on the old Geforces from 6n00 to 9n00, and the "tens" digit for the cards from the GTX200s onward, for which the "30" is equal to a "500", and a "40" is equal to a "570". If you want to carry it farther, the "50" and "650" are about the same, and the "60" is about like a "670".
All that truthfully is beside the point, which is that the latest Geforce drivers are generally not right for old design video graphics cards, and you should always set a RESTORE point before gambling on a new Geforce driver, so you can undo it when once again, it's a bad idea after all. I am guessing now that you already had too new of a driver before now.
Besides the driver, your Direct3D could also be defective (Dx9).
