The vaults weren't designed with the mindset of an actual nuclear war being a likely thing. They deliberately made just a couple of vaults as experimental shelters to test people's reactions to various things.Or else they would've built thousands of vaults. As for the Boston assumption, I think it would make sense if Fallout 4 contained at least 7 and at most about 10-15 vaults. Some would be numbered lower that in Fallout 3, while most would be between 101 and 122.
Just taking a cheap shot at some random Vault numbers:
Vault 89, Vault 95, Vault 99, Vault 110, Vault 111, Vault 117, Vault 118, Vault 120 and Vault 122. That's 9 Vaults, which should provide more than enough exploration for the player, while allowing for some players to add their own vaults into the game through modding.

i would liken that type of instance to dying in game/failing a mission. not gonna be canon
I wouldn't either, but just pointing out that you had the option of not starting it all all. and there's really no reason to think that ending is any less likely than starting it. personally, I doubt they'll link the two games. they're going to do something new. might use some things that were mentioned in FO3, like the androids. but at the end of the day they're going to wan freedom to do whatever they want without having to worry about continuity with the last game.

and considering the whole endgame point is to start the purifier. i'm not sure how a no start ending is likely in the slightest.
they'll continue it in the overall sense, but they aren't going to make a sequel to FO3, it's going to be it's own game, not fallout 3 part 2. the same way they brought the enclave back in FO3 despite it basically being destroyed in FO2. and the whole endgame isn't to start the purifier or you wouldn't have the option not to
Would be cool if they added Vault 999 to the game. At first, every Fallout lore critic would flame them incessantly for breaking the canon, until they actually played the game and traveled to the vault, and then they'd discovered that it's actually just Vault 99 but with an extra "9" sprayed on it, because it's occupied by Raiders who just felt like renaming it Vault 999 because it sounded better. And Bethesda would add it to mess with everyone's head - 'cause let's face it, some of these hardcoe lore fans do tend to think a little too meta-game or otherwise inside the box.

Maybe they could even add a new type of characteristic vault logo to the game - "Vault 999" (with the first "99" being traditional Vault-Tec font and the last "9" being a spraycan font).
That might make more sense, since raiders are into beasts, brutality and all that carnal lovin' found on YouP***. And if Caesar's Legion is involved, then one should also note the fact that Caesar Nero was the most likely candidate for John's mention of 666 in the Bible. Plus, Vault 66 is still left undiscovered, so it could easily be something they added. Only problem is that 66 might be a bit low for the East Coast, lore-wise (again, assuming that Boston is now more than just a hoax). I'm not sure, but if not then Vault 666 could be one heck of an eyecatcher for the game, I think.
