I think New Vegas would have felt a lot more exciting if the battle at hoover dam was set to happen no matter what at a specific time and whether you were there or not. The outcome would be based on things you had or hadn't done.

Yeah...Just say no to the doomsday clock. The best part of Bethesda games is unstructured wandering that rewards you for travelling off of the beaten path. A sword of damocles floating over your head if you don't rush through the main quest as fast as possible is the complete opposite of that.
Off topic for a second. Did you know that the actual saying "He took an arrow to the knee" Is actually referring to falling in love and getting married in old nordic times.

As for this. YES! I would love to see a feral ghoul crawling at you in an attempt to eat your brains. It would add that little hint of realism that would make the game more immersing.
