I have a bunch of ideas, so I'll get started.
1. I know it's post-apocalyptic but civilization has had 200 years and not much has changed, all I want from this is more NPC's around, or at least, the settlements to be busier and more urgent. Take Megaton (using Fallout 3 because I imagine Fallout 4 will have more in common with that than it will Fallout 1 or 2) as an example, they claimed that there were no spaces to live, and it was crowded, yet there were barely 20/30 people in the entire place, I want to feel the claustrophobia and urgency of it all, I want it to feel more apocalyptic and stressful. I want to actually feel the urgency for things like water and food, not two random encounters of people asking for some water throughout the entire game. Simply put, make the settlements busier, make the cities more like cities, people have had time to re-inhabit places, and don't just make them all raiders.
2. A different species. I've thought of two ways you could start the game as either a ghoul or a super mutant. The ghoul beginning being: you could be a vault scientist, working with the reactor for extended periods of time, the humans will never have seen a ghoul before, so when you are fully "transformed" they banish you into the wastes out of fear and disgust, leaving you to fend for yourself, looking for any sign of life, or settlement. This could either lead a slow and side-quest orientated build up, with good knowledge of surroundings and obvious tension, or you could settle into somewhere, be sent on a routine mission and be captured by a faction (a new faction) who capture you after finding out you are a scientist and require you, or simply imprison you, you have to escape with someone, blah blah blah things get interesting. The super mutant is something I would love, basically the game could start with you just waking up, looking around and seeing you're almost on an assembly line, and you can't get out (note you're still human at this point). You're then carried through what seems like a factory of humans, and you hear screams and see super mutants and various other abominations, listening to their conversations on the way. From listening you'll hear the names of other factions and major characters, the only recurring faction I would accept is the BoS as they've been in it for so long, but a new, more powerful style of Enclave, or a new The Master style programme, possibly even one super mutant overlord controlling more intelligent strains of super mutant. As you reach the end of your journey you approach a huge tank of green fluid (FEV to anyone whose played any Fallout ever) and you are submerged. your health drops and your rads go off the chart but you don't die. You feel the controller shake but you can't tell if it's just your character shaking and dismiss it, but in reality a new faction will have broken through and will be in a battle with the super mutants, you are pulled out of the FEV, and you weren't in there for the right amount of time, so you're not full Super Mutant, just half way there. You retain a lot of the facial appearance as you would have set when a super mutant confirms your identity along the way of the assembly line, but your skin has changed, and you look like a slightly smaller Marcus. Obviously from there you talk to the people who freed you as you recover, and they'll eventually set you free once healthy.
Although that may seem very linear, that's just how it starts, and you can go anywhere from there, you would have to deal with the bigotry of the wasteland, the benefits of rad resistance, etc. It could be very, very interesting.
3. More Vault communities, too much exploration of dilapidated Vaults has gotten tiresome, I want to meet some communities.
4. I like what others have said about being able to incapacitate people through decapitation, being able to shoot someones lower leg off, giving you the option to finish them off, or interrogate if necessary, or just leave to die, all for different karma or something, also if that happened to your own faction, being able to heal them.
5. More weapon modification, not set things, I want to be able to put a knife on the handle of a pistol if I want, also allowing you to use your guns as melee weapons for up close and personal situations would be a lot more fluid than switching to a melee weapon.
6. Make V.A.T.S a treat, in other words, you shouldn't rely on it, you should have to earn action points through other kills, or they should just not regenerate as quickly.7
7. Make clothing more diverse, split it into 3 sections if necessary, legs, torso and head, that would be a great feature.
8. A car should be able to be built, just like Fallout 2, either that, or an above ground mono-rail that you can repair. Something that could explain fast travel or avoid walking through those pesky tunnels.
9. New factions, I don't want the Legion or anything regurgitated into this, give us a new environment, New York, San Francisco, Chicago, whatever, but bring new factions with it.
10. Make speech more fluid, the person shouldn't be so static, bring more life to the animation when speaking.
I'll stop now because I've gotten hugely carried away and have rambled.
