Yeah... I could see some problems with that setup as a main gameplay element. GTA sort of works because it takes place in a densely populated and rather expansive city. (Where the map is large enough that really the only way to properly move around in it is with a car; and where it's populated enough that you don't have to go too far to find one. Nothing worse in those games than getting stranded without a car in the middle of nowhere and having to hoof it back to civilization.)
There are working vehicles in the Fallout Universe, but they are also generally pretty rare. I could see it being useful to have a hijacking mechanic you could use for when you did find yourself close enough to an operational vehicle. And you could concievably set the game in an area of the world where cars are a more prominent feature, I should think. (Not saying to go overboard with it, and have the game map littered with operational cars and such - more of a Road Warrior-esque sort of setup.) Maybe areas of the Mid-West that wasn't hit as hard by the War and had more of an access to working vehicles...
But yeah, if we're talking about something close to GTA with a Fallout-y re-texture - that would be pushing it.
