The only good way I can think of that would allow the player to make visually custom weapons inside the game, without getting ridiculous, would be to use a variety of different parts that the player assembles in whichever way he pleases to make his weapon. So in the case of a sword you'd have a variety of grips, guards, blades, pommels and decorations that you pick between to create a sword of your liking. The textures and statistics of the weapon would change depending on what materials you used in your assembly, so you could for instance deign to make a sword with a glass blade and a grip of ebony with a emerald set in the pommel, all visually effecting the look of the weapon. It would require weapons to be not only one model however but a composite of model parts. Such a design would also limit the uniqueness of looks in weapons, because the shapes would have to fit each other seamlessly without clipping or gaps.
Personally I think it'd be neat, but perhaps it would be even more interesting to bring such a system to armour crafting as well. I guess it would be something along the lines of the system found in Neverwinter Night's toolset, and armour and weapons crafting in-game.
