Starts off at 60fps and gradually decreases until 10-20fps. Worse when enemy AI present.
I've done the FALLOUT.ini tweak for quad core.

Monitor your system. Check ram usage, CPU usage, ETC. If you are slowly losing FPS during play, then something is bottle-necking.
