Edit: In seriousness (which is apparently a word.) Caesar's Legion, The New California Republic, or Mr. House, Plus a bunch of small factions that you can gain rep towards or against.

Fractions are ok, I guess, but they don't really help. Anyways, 99/100 is a good fraction, but the well known ones are 1/2, 1/4, 3/4. 3/10. The BEST fraction is x/y.

Anyways, serious answer. Ceaser's Legion, NCR and Mr, House, or you can go solo. There are also other smaller factions around, but I don't think you can join them.

Apart from the main factions you can also join many of the smaller factions, and i think you can still do story things with the smaller factions....
