V.A.T.S Vs. Free Range

Okay so its been a while since I have fully invested into a new FO3 game and I've come upon this question. Most of my past experience with the game I loved and used V.A.T.S. mode to vanquish my enemies but this go around I am trying a more tactical approach so I ask... Why does a single shot in V.A.T.S. (no matter the body part) do more damage than if I shoot out of V.A.T.S? Is it just game mechanics or something else? I know my free range shots are less accurate but I've noticed the discrepancy in a non sneak unaware targeted shot too. (And if the target is not moving, how is it not a straight shot?)
Also, how is it a drugged up raider has such a high perception and can spot me a mile off? Is there a mod or trick to lower this? I'm currently using a mod that removes the Actor tick marks on my compass and find it somewhat annoying.
