Lich. Or rather, the ability to become one akin to becoming a Vampire or Werewolf. The "third race" is the current 9 playable races[adding the Maormer would be nice perhaps]

The Worm cult is the mortal muscle of Bal, and the easiest to work with, as they only require unique face and body paints exclusive to the Worm Cultist that we see opening the anchors and what-not, which is available to all the current playable races already. They would just need to "unlock" the unique paints and stuff for a race aligned to this new faction.

And now, a few posts on this subject from another thread, talking about possible features, and counter-arguments to "lore-breaking."

The mazken maybe? Old boy Sheo made them of course, but they are kinda like mercs. So they might serve for your idea.

The Mazken are well known for serving Mehrunes Dagon during Battlespire too. Definitely possible.

true vampires arent a race so to speak but a vampire that was gifted by molag himself vs via the passing of the infection.

You might call this generic in the MMO world, and I don't really think it belongs in TES lore, I'm not sure

I always love seeing cute lil guys in the game lol like those ones from Final Fantasy, and WoW's gnomes / dwarves / goblins.

Some sort of Atronach could work, I think, as others have pointed out above. A fourth faction allied with Molag Bal has the possibility of being very cool and non-lore threatening if done well.

I'd, personally, squeal with glee if we could play as a Dremora.

While it would be cool to play as a Dremora I don't see the Atronach thing happening seeing as they don't use weapons ever. But I kind of hope they bring in Akavir before they bring in a Bal faction. Maybe even let us go to Akavir.

I thought so and it would be decently easy to do lore wise. I've always wanted to see Akavir and see what other weapons and armor they have besides Katanas. They could even add in a new Samurai type class with this that is mostly tank as they would get their damage from their weapons and skill with them.

Also I read that the Dwemer disapeared , that doesn't necessarily mean extinct.

There isn't much of a way for that character to interact with the game world though. A hybrid dremora/human or elf would be more feasible. Similar to tiefling's in D&D

Yeah lore wise a lot of things are open in this era, except Dwemer technically... although it would be within lore if, in our adventures, we ran into Yagrum Bagarn. Also even though Snow Elves are considered to be a legend at this point The Chantry of Auriel did exist and was accessible during this time. Kothringi left Tamriel around this time because of the deadly Flu going around so it wouldn't be a stretch to see one of them either... a lot of things are open.

@zardoz agreed, after rereading their lore if we don't run into one it would almost be weird... because they do have ties to The King of Worms. Also Imga would be cool.

Perhaps. I sort of figured a Bal faction would have it own territory, like the Covenant or Pact for example, where we'd run around doing quests for pve. With the occasional foray into Cyrodiil for pvp. It has fissures and such opening, right? 50 plus could see us diving into the other factions areas like the rest get to. It could be made to fit into the world with enough effort and skill put into the story.

As prisoners, we only saw the prison sections of Coldharbour. Who's to say what the rest could look like? Plains, seas of some sort, dead forests maybe, deserts, we don't really know. Coldharbour could be some vast place outside of the prisons.

That's what I was thinking. The Dwemer would probably be neutral because of how advanced they are but i'm not sure about the Falmer.

Studying the Snow Elf lore I'd say that they'd probably be on the Altmer's side if they had to choose.
