My game crashes during gameplay pretty much every time i'm playing. It mostly happens just after fast travels when i arrive to the destination. Can anyone help?

Not really. It would amount to nothing other than a WAG, and therefore useless. You failed to provide information. If you were asking about the console version, you could get by with being so reticent about the data, since all consoles are all just alike.
