Episode 2 of Wolfenstien 3D. I can rip through that episode in less than 5 minutes. I've played the whole game countless times, but ten-fold more that particular episode.
Not considering multiplayer, Civilization IV and Doom. No question.
Including multiplayer, WoW.
Played most: Oblivion, no contest from any other game.
Completed most times? Dunno, may well be Manic Miner, or maybe the first Mass Effect.

Kerbal Space Program (if you like space with real"ish" physics, keyword "KSP and Scott Manley into youtube and you may have found a new game to play)
X3: Albion Prelude & Terran Conflict (would love to play these two even longer but the game (or possible my computer) lags to about 6 frames per second after the game has been running for awhile)
Probably a lot of others I forgot about as well.
