I was playing Fallout 4 on the Xbox One, and experienced a major bug. I had just finished the Dangerous Minds quest and then decided to do the Silver Shroud quest. While doing this quest I headed to the Memory Den to talk to Kent but every time I enter the memory den my character a bunch of stuff like chairs and NPCs like Irma fall through the floor. I thought maybe there was just an issue on this save file, but I had an earlier save in the Memory Den and the same thing happens. Even though when I had been in there before on that file the game was fine. I loaded a very very early save from 12 hours ago and the memory den was accessible so I think I may be related to the Dangerous Minds quest. All buildings other than the Memory Den are fine to enter without any problems. I understand a good portion of the main quest takes place in the memory den which I have learned from friends so this is a potentially gambreaking issue. I would lose a lot of time spent playing the game if I have to restart or reload a file from 12 hours ago. I hope you're able to offer some assistance. Thank you.

This bug is in fact happening to me as well, however it is happening at the near end of the game when you go back to the Police Station with the Railroad (for a reason that is a spoiler) and I fall through the floor this is stopping me from completing the game. Any help would be useful.
