I haven't tried it yet but I'm going to take a stab in the dark and say that you won't have that issue when the grenade wouldn't always get thrown (so the quick grenade throw key wouldn't be that quick since you had to hold it down). Just speculating though.

Alex PMed me about this, a new and heavily improved version of the Grenade Hotkey is already done, will be released once NVSE is out of Beta.

:rock: Yes, this is one of my very few must have mods from FO3. The game hasn't felt right without FPS Style Grenade Hotkey.
