I somewhat began to understand it. I don't remember it very well now, but you had to build decks of certain numbers, I usually went with a deck of 24,25,26 and seemed to beat out my competition by placing cards in their caravans making them go over or under. It was just a mini game to me, much like the gambling on the strip. I won 30 hands just to grab the achievement for it, but if there wasn't an achievement for it I would have played it once and would have seen how awful the pay-off is and I'd never have bothered going back to it.
Honestly, I don't see the point in playing in the casinos or playing caravan when I can go explore, find loot and check out the different perk combos, dialogue, quests and endings. But I do think it's necessary, sure it looks like most people didn't really bother with it or enjoy it all that much. But it adds to the culture of the Fallout universe, it gives it more substance. We get to see the inhabitants of Fallout do more than just drink and do drugs for fun.
