Is it a glitch?
In Hell's Kitchen map I was hiding under the stairs. Proximity alarm on max, hunter is nearby. I can't see him, 'cause he is in a front of the stairs. Suddenly I'm dead... What a surprise to watch killcam! According to it I wasn't under the stairs but above it on the landing... 8-0.
First image is a screenshot from killcam record. Second shows where I really was in a moment of my death.
I don't think the hunter was a cheater and ?n general the problem is not important for me but could be for someone else. It happened to me also on East River map.

It has happened to me under stairs in Museum. I mean stairs not ramps. It wasn't a lag problem as I had been there awhile.
