Well weather or not it's appropriate for The Elder Scrolls is anyone's opinion.
I personally don't agree at all with marriage but I'm a bit interested in a sort of unconfirmed romance, like in Half-Life 2. A kid is right out of the question as raising one to an age where you could actually talk to him/her would take at least 5 years in game so it's kind of impractical.
In any case, it's take a lot to make me actually feel something, as in anything other than frustration, for an NPC. The best any game has ever done in this respect is the aforementioned Half-Life 2, especially Episode 2 with Alyx Vance, and that was a LINEAR FPS, meaning that if Valve barely got it right with a lot of pre-scripted behavior and just enough non-scripted gestures to not be repetitive in 10 hours of gameplay, I doubt anybody will make it work in a sandbox game of indefinite length. Then there was Rainbow Six Raven Shield but the only reason I cared there was because, essentially, my team was my weapon son not letting them die was a pretty high priority for me.
Anyway, my point is, if they don't make these would be companions authentic (and/or useful, in a creative way though) enough, they mind as as well scrap the idea and let modders do it, at least in my opinion. And judging by how "well" NPCs were done in Oblivion, Beth should include either better effects for alcohol or anger management classes to go with.
P.S. as previously pointed out, the "optional" argument doesn't stand as development teams don't have unlimited resources.
