I think the face could be from http://planetelderscrolls.gamespy.com/View.php?view=Mods.Detail&id=5883, but there are many face-replacers so it's hard to remember which face comes from which mod.
The http://www.gamesas.com/?showtopic=994490&hl= might be a good place to track down that specific head/hair.
