Your question is a little broad, it might help if we knew more of what you really want from your gaming experience but http://www.gamesas.com/index.php?/topic/1056342-mezs-gameplay-and-immersion-mod-list/ is a good place to start looking.

I use FWE and MMM. With FWE's overhaul and MMM's increased spawns and creature changes I find battles to be very much more like battles. Fallout 3 vanilla is VERY boring compared to my game now.

Well, here's what I'd do . . . especially if you want the fear and loss. I'd play FWE with MMM, and then also use all of TalkieThe Toasters "compaion" mods, namely RobCo Certified, Wasteland Whisperer, and Companions Share + Recuit. If you char isn't the hands on type, you can use those mods to a recruit a little army / band of followers that you can lead around to fight the hordes of MMM. There will be loses.

Well if your looking for a Fallout feel I would use FWE+MMM like others said then change the damage output to 3 (3times the original Fallout3). This is one of the only times I had to run away when outnumbered or facing a behemoth that can one shot you.

If you don't care about it being the Fallout Beth created you could try these mods that make it a zombie apocalypse which makes things pretty intense:

Zombie Apocalypse - http://www.fallout3nexus.com/downloads/file.php?id=10346 // Main mod for this
RTS - http://www.fallout3nexus.com/downloads/file.php?id=7070) // Build your up your fort and try and hold out in your little slice of the wasteland
Apocalypto - http://fallout3nexus.com/downloads/file.php?id=7478 // Makes it much more atmospheric
Ghoul Animaton - http://fallout3nexus.com/downloads/file.php?id=7650 + http://fallout3nexus.com/downloads/file.php?id=10281 //Turns ghouls into shambellers.
Nazi Zombies - http://www.fallout3nexus.com/downloads/file.php?id=7172 // Tough nazi zombies
Zombie Menu and Loading screen - http://www.fallout3nexus.com/downloads/file.php?id=11834 // Haven't used this but should go well with the rest.

These mods really change up Fallout3 which can be a good thing if your bored.
