I've tried everything, but the app never finds the game. Where can I find instructions? (It is on the same wi-fi).

Turn on the Pip-Boy app in the Settings on Fallout 4.

Try to then rescan for devices, if this fails, restart Fallout 4 on your PC or Console and it will pick it up.
