When I reinstall, I go through and...
1) Uninstall all OMODs. That eliminates a large number of files right off. Be sure to delete all the files in the OBMM/Mods folder, too.
2) Uninstall any exe files, like UOP, OOO, Ivellon, etc.
3) Uninstall Oblivion.
4) Delete the contents of the Data folder entirely, along with any other files in the Oblivion folder (I wouldn't delete the Oblivion folder itself, because I keep all my mods backup copies in a subfolder), and all files in My Games/Oblivion.
It's worked great every time (I've done it 3 times now, I think).
