I don't know that I have actually mantled any of my characters with a title outside of them becoming Archmage or something, in-game.
As for the names themselves, it can vary. For the most part, I like to remain as lore friendly as possible, with an allowed window of freedom per RP aspect. I, more often than not, will try to come up with a small phrase that best describes their personality and use Google Translate to find something that sounds lore friendly and tweak the spellings until it looks and sounds right.
I may look to the name history of previous installments for lineage inspiration, should the given character's backstory support it.
Sometimes a name may come to me and I will build the character around that.
Depends on the situation. For an RP, I like to chose my race first, then create a lore friendly name for her/him. For DiD (which I admittedly just started doing) I'll probably just keep going with random names, since they probably won't make it very long in Skyrim.
Occasionally I try to come up with one similar to the OP's...but only if I intend it to be a dry run or jack-of-all-trades type.
My girls never have titles because they're just normal people. None of them are ever Imperials or Bretons; most of them are half-elves, a couple have been Nords. I don't play Orcs, High Elves, or Bosmer. I've got a couple of Redguards and Dunmer around somewhere. But no matter the race, they're all just normal people. No titles, no "hidden fancy backstory". Just women caught up in things that happen without their intention or input. Some of them are Dragonborn (because I like to play that DLC) but I always use Skyrim Unbound so it goes "my way". None are ever werewolves or vampires.
Common garden-variety women, just trying to stay alive....

My character names come from a work of fiction I've been working on for quite a while. Playing games like Skyrim gives me ideas for their stories.
Buffy the Bowgirl. The last bit is so she doesn't get confused with other vampire slayers who might share the same first name.
Most of my characters just have names. Sometimes the names are descriptive, like my Argonian "Yellow-Boy," who is both bright yellow in color and an abject coward. Or my evil character Penelope, who is nicknamed "Penny Dreadful."
I've had a few with "titles."
My current DiD competition character is "Vixi, Ornithog-Rider #13," because she's ranked thirteenth among the Akaviri gene-modified-cliff-racer riders. (She's also the 17th daughter of the Nerevarine.) 17+13=30, which has no bearing on anything at all.
My "Barb-Ann the Contrarian," a Nord who started life as a college basketball player, was hit by a beer truck in Boston, and knocked into Tamriel. She doesn't believe in the reality of Tamriel, thinking it all a dream, so she doesn't take anything seriously, and drinks her way from one failed quest to another.
"Fausto the Orc Wizzard" (his misspelling of "wizard"), is an orc brawler who took too many blows on the head, and came to believe that he's a mage. He always casts a weak and useless flame spell before bashing his opponents with a big hammer.
I don't think she's on my PC any more, I'd have to search the archives for her save.
Just picture a black-haired Breton in the Savior's Hide, with Dark Brotherhood boots, gloves, and hood, and the Rueful Axe strapped to her back.

Mikke of Green River wanted to make sure people knew which Mikke she was, so she called herself by the town she left behind. I thought that Skyrim needed a Green River so I made one up for her.
Captain Zanimal was the captain of his ship (mod).
One-Eyed Willem was named for his most distinctive feature.
Njal the Unliving got his name after a botched resurrection left him undead.
Marco the Thief thought that sounded better than Marco the Liar or Marco the Murderer. He was all of those things.
Everyone else that I can remember at the moment just has (or had) a surname if culturally appropriate.
Surely none of our characters can hold a candle to the awesomesauce name held by Erik the Hoe-Pusher!
I usually give lore friendly names to characters based on their race. I did have one Argonian named Reptard. I loved Reptard.
Like most here, my characters do not have "titles." However, the game enjoys giving them nicknames, especially if one joins the Stormcloaks
I may also pick up a few titles along my journeys, like:

Guild Master
Harbinger
Captain
None of my characters have titles. But i have been given the title "the Inept." I'm still trying to live down the day Oblivion came out and I repeatedly tried to walk through the Oblivion Gate at Kvatch. "It's not working," I said. "I think my game is bugged." "You have to click on the gate, Pseron," sighed my patient, long-suffering Significant Other. I think she was already half finished with the main quest by that time.
Depends on the character? Although really only Gilliam has a title out of all of my characters. "General Gilliam".
Most of my guys tend to be simple folk trying to make a living. Unless they are part of that storyline, then they are simple folks who keep getting shoved into Deadric and Aedric adventures.
And.... that would make three of us! Happy to be in such delightful company.... and my husband, who knows nothing about this sort of games, or about computers, the internet and so on, was no help at all as I was making myself bald....
The only character of mine that had a title was Tzirra the Timid. She was a sneak thief, who eschewed all forms of combat. It was a very fun and challenging character. I lost her save when my game got corrupted by mod scripts run amok.
Most of my characters don't even have last names, let alone titles. I should think about that for the future, though. Usually, a character will get a last name if they are nobility or otherwise "important" in the world of Skyrim. I don't play too many of those characters though.
The exception was Solveig Fox-Tail. I had high hopes for this farmgirl-turned-Dragonborn. Alas, she was smashed by a frost troll, high on Hrothgar, before she could finish shouting it off the mountainside. I was very sad about that.
