The Institute does have a fabulous medical plan.
You should see a big red medical sign above one of the facilities, and it has a number of medical beds in it. That's the medical bay. Dean Volkert is the fellow you're looking for. He's wearing a white and green lab coat and is usually found at his terminal.
Tbh, I haven't even reached the Institute yet in my current playthrough and I already have over 200 stimpacks on me, so, no. But the free instant-travel is a nice boon, and synth relay grenades are fun to spam. Plus X6 is best companion, so they have a lot going for them regardless, afaic.
I just hope I could do something about MacCready's teeth. I can't even imagine his breath.
Let's not forget the toilet paper, hot showers and clean towels. After a hard day's work above ground with the filthy peasants of the Wasteland, it's nice to clean up, grab some bourbon and bet caps on some synth gorilla cage fighting.
Refreshing beverage should do the trick.
The doctor doesn't ask too much too. Just talk and wham! Rads gone, health replenished.

I feel like he is bugged for me, as he mentioned doing an exam on me and never actually doing it. Just stays on his terminal all day. Can still offer his services though to cure me of any ailments, so that's good.
But besides that, between the ability to teleport, X6-88, synth relay grenades, a beautiful underground utopia to call home, the marvelous scientists, toilet paper, and of course the preservation of humanity's future, I find there is plenty of good reason as to why one would join the Institute.
I don't think he actually does the exam. Its one of those "background" things. Like Dr. Watson asking to meet with him later to discuss policy updates, which you also never get to see him and the SS do.
Ah, alright then. I assumed it was going to be something similar to what the Brotherhood medical officer does, but I suppose not.
Currently playing a high-endurance build with lots of Lifegiver, Solar Powered and Ghoulish perks.

Still use the occasional stimpack in combat, although cooked food is generally a better option: bonuses and no animation when I apply them.
I really should sell most of those stimpacks, thinking about it.
