Also, if they are hidden, how do they know they even exist or that people have earned them? They are HIDDEN, after all.

Though, I know a couple of people who work at Valve, specifically with Steamworks and the Steam Community features. They assure me that Steam doesn't have hidden achievements for any game. There are hidden statistics, but not earn able achievements.
