I, myself, am entirely for realism. When you just start out, there are tons of things out there that will straight up end you. No questions asked. However, once you become leader of the Fighter's Guild, Archmage of the Mage Guild, etc. etc. etc, there is going to be a very few things that will Gandalf you. However, to increase gameplay, there still should be some things that put up a very decent fight. There should be some enemies that consistently level scale with you, no matter what. My brother is one of those obsessive players that will level up religiously every chance you give him, and will only go after bosses when he's sure he will annihilate them. I think there should be some baddies that go up when you go up. Realistically, they could've been out there, fighting good guys and leveling up just as much as you did. But like a previous poster said, there is simply no way a goblin tribe should be able to pose a problem for an epic hero. I'll end my rant here, as I'm sure this is entirely too long for the attention span of most.
