Lily's unmarked quest.

To trigger the quest, one must open a dialogue with her after hearing a recording of children greeting their grandmother.

I am almost at the end of the game yet I still don't see any sign of this. I am aware that I must have her as a "active" companion, but what does that mean? Also if there is a console command for this, please let me know :hehe:

Did you ever get a dialogue option with her where, you seemed to be telling her which dosages of her medication to take?

See I got that, and I think that IS the hidden quest? but I don't recall hearing the grandchildren recording?

The grandchildren recording will happen, at which time at any time afterwards you can innitiate conversation with her and tell her when to take her meds. As far as I know there's no intentional way to trigger the recording. Mine happened when I finished sleeping, I hear the recording and talked to her about it.

To trigger this you must have Lily get low health 3-5 times while fighting something.

All I did was allow her to fight stuff and not help her and once she got low health and the steps trigger for the quest, then heal her back up with stimpacks.
Just do this 3-5 times and then wait a bit and you should hear the recording of her talking with children and that is when you talk with her and can finish the quest and tell her how to take her medicine.

Ah yes, now I recall reading about that. And by low health, he means until you get the popup message about her going berserk I think.

So I've been trying just this, and I've done it at least five times now - gotten her to low enough health in combat that she has her "psychotic break", then waited for it to end, then doing it again. But still, nothing appears to be happening afterwards.

Another thing - I'm not getting any popup messages for her phychotic breaks (apart from for her very first one). I know she's having them because I have her constantly set to "passive", and she becomes "aggressive" and I can't change her back for a while. And then there's the audio clues - her aggressive roar when she enters the break, and comments like "Oh, did I do all that?" and "Gracious me, I lost control" when it ends. Am I supposed to be getting popup messages for it, or is this normal?

Okay, I must have triggered at least 10 psychotic rages by now, and still nothing to show for it. I'm really hitting a wall here.

One time explaining message is the only one you are meant to see, from waht I can tell.

Hmm. Well, in that case I'm stumped. It's especially frustrating since I do distinctly recall getting the "how much medicine to take" dialogue in my very first playthrough, despite not having Lily as my active companion for that long. I also don't remember even hearing the recording with the kids back then, though I suppose that may just have been due to me not paying attention.

Oh well, I guess I'll just try fiddling around a bit and see what happens - maybe dismiss and rehire her, or use console commands to disable/enable or kill/resurrect.
