My bad, didn't read all of the text :blush: After downloading, I can see there's a script attached to Nels Llendo. It's supposed to bring up a messagebox when you interact with his corpse and that's how you get the head.
At a guess you've another mod loading later in your load order that adds a script to Nels Llendo too. If you feel up to identifying which the problem mod is you could try a "single vs all" check using the TES Plugin Conflict Detector (TESPCD).
Or use something like Yacoby's Reorder Plugins to move Living Tower to the end of your load order (or below the interfering mod identified using TESPCD).
Following that I'd recommend updating your save using Wrye Mash:
On the Saves tab, select the savegame you want to fix.
When the savegame is selected the list in the bottom right will be populated with the mods currently associated with the savegame.
Right click the column headers ("File" and "Num") and choose Sync to Load List (this will change the mods associated with the savegame to match those currently selected on the Mods tab).
Click Save
Right click the savegame and choose Repair All.
[Optionally, right click the savegame and choose Remove > Debris Cells.]
The checkbox for the save should now be purple.
