Possible outrage? It would be like a mentally handicapped person complaining that NPC's in Fallout 1 were talking down to them because they purposely created a character with 1 point in their Intelligence stat.
If you purposely put a 1 in your Intelligence stat, the outcome should be your character speaking as a character with low intelligence. It would be a choice the player makes. One that could easily be avoided by not creating a character with low intelligence.
I think it's stupid as well (I love the low intelligence dialogue options in the Fallout titles), but the point is that Bethesda may be wary about including it due to other people (e.g. the media) getting mad about it, not us gamers. They worry about the potential media fallout that could occur for seemingly mocking the disabled. Of course I don't think that will actually be that big of a concern in reality, but that might deter Bethesda from perusing it.
Just because something has been done before doesn't mean it can be done again without controversy from today's busybody media.
Did you know Daggerfall had full frontal nudity? How much do you want to bet the media would be in hysterics if Skyrim had that too (well, without mods I mean).

In all honesty I'm not even sure they'll get large backlash, as it seems determinate on whether they voice the game or not. No one cared about it in New Vegas, so it could get by in F4.
