Are these found randomly or are there places where you can always find them ?

Like all things in Oblivion, its randomly generated in loot.

Though you would want to look in: (taken from UESP)

In Grummite Dungeons
- Boss-type Hollowed Stumps, Chests, and Urns: 1 guaranteed Madness ore, %40 of another, and of this as the player is on level 1-8, there is a 14.3% chance to find a matrix, and after level 9, there is a 25% chance.
- Standard Hollowed Stumps, Chests, and Urns: 25% chance for Madness ore/Madness Matrix
In Zealot Dungeons
- Boss-type Hollowed Stumps, Chests, and Urns: 1 guaranteed Madness, 10% chance for an Madness Matrix

Well...this stinks...I replayed Shivering Isles for the single purpose of finding a madness arrow matrix...I have been through every inch of the map,through every dungeon...I have found every matrix accept the arrow matrix...bunk !

Well, you'll just have to go trough them again. Wait for the dungeions to reset (was it 72 in-game hours?) and check them again. It's all about luck (not the character atribute

Yeah I been through everywhere three times...it is like the game knows I want it and is saying #&$&%* you ! hah.

Just to make you rage even more. I had a character that found that matrix six times with just one lap through the dungeions. Anoyed that the game refused to drop a sheild matrix at the time.

That is how Oblivion goes sometimes...either things always seem to happen the way you want or everything goes wrong for you...
